All undergraduate students taking 9 or more credit hours and Graduate students taking 4 or more credit hours are automatically enrolled in the United Healthcare Student Resources student insurance plan at registration, unless proof of comparable coverage in the State of Florida is furnished on a hard waiver basis. All undergraduate nursing majors, all Division of Medicine, Physician Assistant and Anesthesia are automatically enrolled in this insurance plan at registration, unless proof of comparable coverage is furnished on a hard waiver basis. All registered PACE and part-time graduate and part-time undergraduate students enrolled in a degree program are eligible to enroll in this student insurance plan voluntarily online with UHCSR. Students who are enrolled in programs designated as fully online are not eligible to enroll in this plan.

Choosing to take your classes remotely due to COVID 19, is not the same as being an online student. All students who are required to be insured are to complete a waiver whether or not they are electing to remain remote rather than attend classes face to face.

* Students are eligible to purchase the plan on an Annual basis or for both the Spring and Summer semester directly at www.uhcsr.com/barry. If a student purchases the insurance directly with UHCSR they must complete a waiver to remove the one semester charge from their student account.

** Students who are enrolled in the spring semester but not enrolling in the summer semester at Barry but wish to remain on the UHCSR student Insurance plan, are eligible to enroll directly at https://requestcoverage.uhcsr.com/barry.

Students who have been previously enrolled are eligible to continue their insurance benefits and may enroll in the UHCSR continuation plan at www.uhcsr.com/barry.

SPOUSE/DEPENDENT ENROLLMENT PROCESS

  • Enrollment is available for Spouse/Dependent(s) through www.uhcsr.com/barry. You must be enrolled in the student plan and effective in the UHCSR system before you can request Spouse/Dependent coverage Spousal/Dependent coverage terms run concurrently with the student coverage terms. To be eligible for coverage, students must meet the credit requirements as stated in the eligibility statement and actively attend classes for the first 31 days after the date for which coverage is purchased. Home study, correspondence and online courses do not fulfill the eligibility requirements that the student actively attend classes.

UTILIZING THE PLAN

The most cost efficient and convenient way to access health care on the UHCSR student health plan is to utilize the services in Student Health Services (SHS). Appointments are preferred but walk-ins are welcome.

If you have a condition that falls outside the scope of practice of SHS or you have an existing relationship with an area physician that accepts United Health Care insurance, please:

  • Contact the doctor’s office and schedule your appointment
  • After you have a scheduled appointment please call SHS @ 305 899 3750 to request a referral.
  • Provide SHS with the name of physician, specialty of the practice, reason for the visit and the office fax number.
  • If you are outside main campus area by more than 50 miles, you do not need a referral. If you are requested to have a referral by the physician’s office, simply call SHS and one will be sent.
  • Spouse and dependents do not require a referral.
  • Co-payments and deductibles apply when seeking healthcare from an off-campus provider and are the patient’s responsibility.

INSURANCE REQUIREMENTS

WHAT IS CONSIDERED ADEQUATE INSURANCE?

  • Your health insurance company must be licensed to do business in the State of Florida and coverage must extend to the Miami Dade/Broward county area.
  • Coverage must remain in effect while enrolled for classes.
    • The premium must be paid in full for the semester in which you are enrolled; premium payments submitted on a monthly basis are not considered continuous coverage.
  • Access to primary and specialty care in Miami Dade/Broward County.
    • Hospital Emergency Room/Urgent Care Center only coverage is not accepted
  • Inpatient hospital coverage for Medical and Mental Health issues.
  • The insurance plan must pay for services directly to physicians and/or hospitals.
    • Reimbursable plans are not accepted.
  • Prescriptive Medication Coverage (Unlimited coverage with copayment)
  • An individual deductible not to exceed $2,500.00 per policy year
    • Plans with a Health Savings Account (HSA) and/or a Federal Savings Account (FSA), are exempt from this requirement.
  • Outpatient Mental Health and Substance Abuse coverage.

Note: International plans are not accepted. International students may not waive this coverage unless they have private health insurance coverage with a U.S. insurance plan (i.e., Aetna, Cigna, BCBS or United Healthcare)

WAIVER PROCESS

MANDATORY HEALTH INSURANCE PROCESS:

Students MUST complete the insurance waiver and upload a copy of their insurance card within to the first 30 days of the semester. The insurance cost is nonrefundable after the waiver period.

Step 1: REGISTER FOR CLASSES-This must be done before you can complete any of the processes below.

Step 2: CHECK YOUR BARRY STUDENT EMAIL! Barry email is the official form of communication for the University. You will receive an email 24-48 hours after registering notifying you of the billing and instructions for waiving or activating the student insurance plan. Be sure to read all of the information in the email that is sent to you. You will not be able to complete the waiver or activation process until you receive the email notice.

Step 3: GO TO THE LINK PROVIDED IN THE EMAIL (https://studentcenter.uhcsr.com) and choose either WAIVE COVERAGE to request to have your private insurance considered and the student insurance charge removed from your account OR, ENROLL to accept enrollment in the student insurance plan and activate the coverage. You will need to provide your student ID number to access the link and if you choose to waive the plan you will need your health insurance card. After you submit the waiver, you will receive an automatic response of accepted or declined. You may contact Student Health Services for any questions regarding declined waivers.

IMPORTANT: WAIVER SUBMISSION DEADLINES

WAIVER OPENS: The online waiver process will be available from the first day of registration for the semester in which the student is registering (fall or spring), through the first 30 days of the registering semester. Students beginning classes in the fall semester must complete the insurance wavier each subsequent fall semester. New students beginning classes in the spring semester will be required to complete a waiver in spring and again in the fall of the same year and each subsequent fall semester. PLEASE NOTE: Students will not have access to the waiver process until they have been registered. Students will receive an email notification with the link to the waiver site when they are able to complete the waiver process. You must check your Barry Student Email!


HEALTH FEES AND BILLING
STUDENT HEALTH ACCESS PLAN

All Full-time undergraduate students attending class on the Barry University main campus (Miami Shores) and all graduate students residing in University provided housing will be billed for a mandatory $95 Health Fee. This provides for primary care services at the Student Health Center through the Student Health Access Plan and coverage for up to $5000 for accident coverage through the Student Accident Plan. This billing is done on a per semester basis, once in the fall and once in the spring. All other students may purchase the plan if they wish to utilize the primary care services at Student Health Services. Coverage highlights for both plans are listed below.

THIS PLAN PROVIDES THE FOLLOWING COVERAGE:

  • Unlimited visits for evaluation by a Registered Nurse during SHS hours of operation.
  • Unlimited primary care visits with a Nurse Practitioner for the semester during SHS hours of operation.
  • Select over the counter medications in limited quantities.
  • Select prescriptive medications (high cost medications may have a nominal co-payment).
  • Immunizations are not covered for students with primary insurance other than the Barry Sponsored UHCSR plan. The cost for immunizations at SHS is discounted below community standard pricing and is billed to the student account.

PLEASE NOTE:

The Student Health Access Plan covers services provided ONLY through Student Health Services (SHS). This plan IS NOT an insurance plan and WILL NOT cover the cost of medical treatment or medications at off-campus providers. It is strongly advised that all students maintain a private comprehensive health insurance plan. Please see the University health insurance requirement at www.barry.edu/healthservices.

This plan covers most services provided at SHS except; immunization, high cost prescriptions and Laboratory tests. If the student has the UHCSR student plan, these cost will be billed directly to UHCSR without additional cost to the student.

  • Laboratory tests must be sent to an off-site lab and will be billed through the patient’s private insurance. Any unpaid balance owed to the laboratory is the patient’s responsibility.

The Student Health Access Plan and Accident Plan are mandatory and cannot be waived for any reason. The Student Health Access Plan will continue to renew for each subsequent semester for Graduate students that remain in University housing. It is the responsibility of the Graduate student to notify SHS if they make the decision not to reside in University housing for the subsequent semester.

PROCEDURE FOR FILING AN ACCIDENT CLAIM

In the event that you have suffered an injury that requires medical attention, please follow the procedure below:

  • Occupational Exposure, (needle stick or bodily fluid exposure)
    • Wash the wound and skin site that have been exposed to blood or bodily fluids with soap and water. Mucous membranes should be flushed with water.
    • Report your injury to your clinical supervisor immediately!
    • Immediately seek care by a qualified health care provider(ER, Urgent Care or a health care provider that can provide appropriate guidance on the need for antiretroviral prophylaxis)
    • DO NOT SEEK CARE IN THE STUDENT HEALTH SERVICES for Occupational Exposures!
    • Follow all federal (OSHA) and state requirements for recording and reporting occupational injuries and exposures as per the clinical site and the Barry University program guidelines.
  • For any other injury (sprains, fractures, lacerations, etc.) that requires medical attention:
    • Please seek medical attention as soon as possible at an ER, Urgent Care Center, Private Physician’s office or the Student Health Services.

If your injury meets policy coverage guidelines, please contact Student Health Services at (305) 899-3750 as soon as possible after medical care is received to schedule an appointment to complete a claim form. If you are not at the Barry University main campus please contact Student Health Services to request a claim form.

You will be responsible to provide the treating facility with your primary insurance card and your accident insurance card. The accident plan will cover up to $5000 of medical expenses per injury, which is NOT covered by your primary insurance plan. (Exclusions may apply) The accident plan is a supplemental plan. You must have primary insurance that has been approved by hard waiver as per the Barry University guidelines.
