• Application Forms

  • Application Fee Waiver Form - The nomination form for alumni to recommend potential students for admission to Barry University. This form will waive the application fee.
  • Authorization for Release of Application Information - For use by applicants who wish to permit an agent or representative to receive information on their application status.
  • Change of School/Program Form - For enrolled students who wish to change from one school or program at Barry University to a different school or program at Barry.
  • Graduate Readmission Application - To be used by students who have been inactive in their academic program for one calendar year.
  • Non-Degree Application - For applicants wishing to enroll as guest students, non-degree students, or those pursuing teacher (re)certification.
  • Nursing Admission Statement Supplement - Essay questions for applicants to the MSN, DNP or Nursing PhD programs
  • PACE Graduate Continuation Form - For use by qualified UG PACE students continuing on to the graduate level in a PACE program. Use with advisor's approval only.
  • Photography Portfolio Review for MA and MFA Degree Program - Must be submitted with Graduate Application for the MA/MFA in Photography

  • Financial Affidavit of Support for International Doctoral Students - Must be submitted by international Doctoral applicants requiring the I-20.
  • Financial Affidavit of Support for International Graduate Students - Must be submitted by international Graduate applicants requiring the I-20.
  • Financial Affidavit of Support for Physician Assistant Students - Must be submitted by international Physician Assistant applicants.requiring the I-20.
  • Financial Verification Form – PACE Graduate Students - Must be submitted by PACE graduate students requiring the I-20.
  • Financial Verification Form - Podiatric Medicine - Must be submitted by international podiatric medicine applicants requiring the I-20.

  • Graduate Recommendation Form - A hard copy version of the recommendation form for the master’s and specialist level programs.
  • Graduate Recommendation Form for the School of Social Work - A hard copy version of the recommendation form for the master’s and doctoral level programs of the Ellen Whiteside McDonnell School of Social Work.
